Carrier Smells Fishy But I Can't Prove It

I've got a carrier that I'd like to use for a load. Rate is good. Dispatcher said all the right things and provided driver info, insurance cert, etc. but something just seems off. Can someone prove that we shouldn't use this carrier and why:
