Tier 4 perk not working

I have craftsmanship maxed out and I'm trying to reforge a sword but it never goes past tier 3. I've checked to see if I have the perk and have taken a potion to increase my craftmanship but nothing seems to help. Can the reforged gear not be tier 4? Does it have to be only weapons you forge from scratch?
